Holiday Mini catalog help - looking for a template/pattern

Hi. I am wanting to make some of the boxes (I guess that is what it is called) that is shown on page 2 of the Holiday mini catalog. It is the Green box at the bottom of the page. Does anyone know where I can find a template/pattern for this?

Here is a pattern for a similar shape. You could modify it a bit to get the exact shape you're looking for. Hope this helps!
