ISO 2012 blank calendar pages

I am looking for a printable 2012 calendar that prints 4 months to a page, which would then fit into a CD cover for display.

Amuse used to offer one for sale each year that benefited a charity ... but I cannot find it for this year.

Does anyone have a link to a 2012 calendar in this format?
