box on pg. 120 SU! Spring Summer Collection?!??

I need the template for the cute box on page 120 of the SU! Spring Summer Collection. It holds those cute little scallop note cards. Anyone?!??? I've tried several that I've found and they just don't fit!! And now I have so many paper cuts that I dare not attempt even one more!
Any help is MUCH appreciated!

Here are two box templates. I have never used either one but you are welcome to try them. this is the first time I have tried to send an attachment so I hope it works.

Also see instructions on SCS for 5 in one template box under resources. If you read far enough it will show you a template for the bottom of the box. You can use this to make a box any size you would like. Hope this helps.
