Incredible Giftcard Holder-Directions needed

There was an incredible folded giftcard holder posted by Faereygirl on Oct. 25, 2006. Does anyone have the directions? I have not yet had a response to my PM to her. (I don't know how to link to her gallery submission.)

Thank you so much for the tutorial you posted on your blog to complete the gift card holder. What a great idea and it works up so quickly. I have made 12 so far with 8 more to go. I will be using these to hold the Lottery Tickets that I give to each of my family members for Christmas.
