Default Need Magic Slider Card Directions

I'm loving the Magic Slider Cards in the gallery - the ones that have a top and bottom section that slide out. I'm not finding any directions very easily! Could someone e-mail them to me? Thanks so much!

Thanks for the directions! I got the template from SU, but had no idea how to make the card! I can't wait to make one today!
